The signing of this application shall constitute an agreement to abide by all the rules and regulations governing the use of school buildings. The applicant also agrees to hold harmless the Town of Ludlow and Ludlow Public Schools for any liability claims resulting from the loss of personal property or bodily injury. The applicant acknowledges receipt of a copy of Massachusetts General Law 269, Section 17, that prohibits the practice of hazing and / will provide copies to all program participants.
The representative of the organization signing below acknowledges receipt of each of the above items. The undersigned has read and understands all obligations of the organization to the Ludlow Public Schools for the use of its facilities, particularly the obligation of the undersigned organization to arrange for police coverage for events attended by more than 100 persons, to provide proper supervision for any event and to be financially responsible for any damage caused by vandalism.
